Medical Director Multidisciplinary Pelvic Health, Albany Medical Center
Dr. Elise De is board certified in Urology, Female Pelvic Medicine and Reconstructive Surgery/ Urogynecology and specializes in Reconstruction, Neurourology, and Voiding Dysfunction in all genders. Her academic and clinical passions include multidisciplinary collaboration, high impact education of professionals and patients, research into complex conditions, and models of care delivery for urinary incontinence, fistula, prolapse, voiding dysfunction, neurogenic bladder, and pelvic pain. Dr. De strives to provide colleagues and patients the tools needed to understand incontinence and pelvic symptomatology in the context of pelvic organ cross talk, and the dynamic interplay of musculoskeletal, neurologic and other contributing disease processes impacting functional outcomes. For example, her multidisciplinary intake form returns to the patient an organized Word document with interpretation of scores from screening validated PROMs at the same time it is uploaded to the clinical EMR. Dr. De’s unique career impact on the field of Urology stems from her combined, multifaceted approach to innovation. She has published and spoken extensively on basic physiology and surgical technique, incorporating and facilitating knowledge from other specialties, has harnessed the power of the EMR and electronic education for professionals and patients in mutual collaboration, and on many levels has performed electronic outreach to reach those in need of education, for example the ICS fistula videos.
Her career efforts in incontinence have forged multidisciplinary collaboration especially in complex care:
• Promoting the value of a multidisciplinary approach to patients with incontinence and pelvic floor disorders. Co-occurrence of pelvic floor disorders is high, with 80% of women with stress incontinence or overactive bladder, 69% with pelvic prolapse, and 48% with anal incontinence reporting at least one other disorder. (Markland A J Am Ger Soc 2008 285-290, Lawrence J Obstet Gynecol 2008 678-685)
• Teaching native tissue surgical approaches to incontinence and prolapse in the wake of mesh.
• Publishing on prevalence of neurologic disorders in incontinence, including autonomic dysfunction.
• Establishing the basic science / clinical relevance of levator dysfunction in approach to incontinence
• Educating worldwide on treatment of pelvic pain, which coexists in 30% of patients with incontinence
• Improving management of neurogenic bladder and incontinence and options in impaired hand function
• Furthering urodynamic technique and explanation of the procedure (https://www.facingpelvicpain.org)
• Elevating management of complex incontinence – salvage prostatectomy, NGB, fistula and prevention
• Programming of sophisticated multidisciplinary patient intakes facilitating outcomes in combined care
Her societal contributions span 20 years - the American Urological Association (OAB Core Curriculum, AUA Courses on Native Tissue Repair for Incontinence and Prolapse, AUA Guidelines on Chronic Pelvic Pain, AUA Update on Chronic Pelvic Pain), the International Continence Society (Chair of the Education Committee 2016-2022, key developer of ICS TV), Society of International Urology, Society for Urodynamics, Female Pelvic Medicine and Urogenital Reconstruction, the International Urogynecological Association, and others.
